Job summary

Community Care of WNY is seeking a licensed Social Worker (MSW) in Allegany, NY to provide direct and supportive social services to patients and families as part of a care plan. You will assess, counsel, coordinate with the care team, and support bereavement services while maintaining compliant documentation.

The role requires a master's in social work, NY licensure, and at least one year of healthcare social work experience. Strong communication and organizational skills are essential.

Qualifications

• Master's degree in social work required.
• Current certification or license to practice social work in New York state required.
• Demonstrated organizational and supervisory skills; strong communication abilities.

Responsibilities

• Establishes initial plan of care for patient and family with RN and attending physician.
• Performs, reviews and approves social work assessments and documents in clinical records.
• Provides counseling and referral services for social, financial, emotional, and crisis needs.
• Assists the team in understanding social and emotional factors affecting care.
• Participates in interdisciplinary team, case conferences, and quality improvement meetings.
• Acts as liaison with community agencies to coordinate social work services and avoid duplication.
• Completes bereavement assessments and coordinates bereavement care as needed.
• Prepares required reports and documentation and participates in staff education activities.
