Job summary

Carle Health is seeking a Home Services Social Worker to assess psychosocial needs of patients and families and collaborate with interdisciplinary teams to coordinate care. Hours vary with census and program needs; travel for home visits may be required.

This role requires Hospice experience of 1+ years and offers a sign-on bonus. The Social Worker will develop care plans, educate families, and connect patients with community resources while ensuring dignified, patient-centered care.

Qualifications

• Hospice experience required (1+ year).
• Master’s Degree in Social Work.
• Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in Illinois.
• Driver’s License and proof of auto insurance.
• Ability to work as part of a team.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and life situation.
• Develops a bereavement plan of care to address family member/caregiver needs.
• Interprets and communicates pt/family faith and culture traditions; identifies how spiritual beliefs may impact care.
• Educates patient and family members to overcome barriers and meet fundamental needs.
• Documents psychosocial assessments, financial assessments and MSW interventions in the EMR.
• Implements social work plan resulting in strengthened family systems, resource utilization, and patient dignity.
