Job summary

The Home Services Social Worker at Carle Health identifies psychosocial needs of patients and families through assessment and supports care planning within an interdisciplinary team. Role requires Hospice experience of 1+ years and offers a sign-on bonus.

Responsibilities include conducting psychosocial assessments, developing bereavement plans, coordinating community resources, and documenting care in the EMR.

Qualifications

• Hospice experience (1+ years) is required.
• Masters of Social Work (MSW) or equivalent degree.
• Current licensure as LSW or LCSW in good standing.

Responsibilities

• Conduct psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develop bereavement plans of care addressing family needs.
• Educate patients and families about resources, supports, and care options.
• Document assessments and interventions in the electronic medical record in a timely manner.
• Coordinate with interdisciplinary teams to support patient goals and comfort.
• Assist with transportation barriers and referrals for essential services.
• Support advance directives, living wills, and POLST forms.
