Job summary

Everwind Hospice in Casper, Wyoming seeks a compassionate Hospice Social Worker to support patients and families with emotional, social, and spiritual care. You will conduct assessments, create care plans, and coordinate with an interdisciplinary team to ensure comfort and dignity at end-of-life.

You will provide education on end-of-life options, bereavement support, and community resources, while documenting interactions per agency policies.

Qualifications

• Master's degree in Social Work (MSW) is required.
• Active, unrestricted Wyoming LSW or LCSW license.
• Strong understanding of social work theories, practices, and ethics.

Responsibilities

• Provide emotional, psychological, and social support to patients and families, assessing needs and developing care plans.
• Collaborate with interdisciplinary teams to ensure comprehensive care.
• Develop and maintain relationships with patients, families, and caregivers, addressing social, emotional, and spiritual concerns.
• Identify and address social determinants of health (housing, transportation, financial needs) to improve outcomes.
• Facilitate communication between patients, families, and healthcare providers for informed decisions.
• Provide education and support on end-of-life care, coping, and bereavement.
• Document interactions and assessments in accordance with agency policies.

Responsibilities

• Provide emotional, psychological, and social support to patients and their families, assessing their needs and developing individualized care plans to meet those needs
• Collaborate with interdisciplinary teams, including physicians, nurses, chaplains, and other healthcare professionals to ensure comprehensive care
• Develop and maintain relationships with patients, families, and caregivers, addressing social, emotional, and spiritual concerns
• Identify and address social determinants of health, such as housing, transportation, and financial needs, to improve patient outcomes
• Facilitate communication between patients, families, and healthcare providers to ensure informed decision-making
• Provide education and support to patients and families on end-of-life care, coping mechanisms, and bereavement
• Document all interactions and assessments in accordance with agency policies and regulations

Requirements
