Job summary

Everwind Hospice in Casper, Wyoming seeks a compassionate Hospice Social Worker to support patients and families with emotional, social, and spiritual care. You will conduct assessments, create care plans, and coordinate with an interdisciplinary team to ensure comfort and dignity at end-of-life.

You will provide education on end-of-life options, bereavement support, and community resources, while documenting interactions per agency policies.

Qualifications

Master's degree in Social Work (MSW) is required.
Active, unrestricted Wyoming LSW or LCSW license.
Strong understanding of social work theories, practices, and ethics.

Responsibilities

Provide emotional, psychological, and social support to patients and families, assessing needs and developing care plans.
Collaborate with interdisciplinary teams to ensure comprehensive care.
Develop and maintain relationships with patients, families, and caregivers, addressing social, emotional, and spiritual concerns.
Identify and address social determinants of health (housing, transportation, financial needs) to improve outcomes.
Facilitate communication between patients, families, and healthcare providers for informed decisions.
Provide education and support on end-of-life care, coping, and bereavement.
Document interactions and assessments in accordance with agency policies.
