Job summary

Baptist Trinity Hospice is seeking a dedicated Social Worker for hospice per diem services. You will provide medical social services under the direction of a physician and the Interdisciplinary Group, focusing on emotional and social factors affecting the patient’s health status and coping strategies.

Key responsibilities include contributing to an integrated Plan of Care, guiding the team to resources, and, when needed, serving as Bereavement Coordinator to address family bereavement needs with

Qualifications

• Master's Degree in Social Work from a CSWE-accredited program.
• At least 1+ year of social work experience in a healthcare setting.
• Current driver's license, insurance, and reliable transportation or access to public transit.

Responsibilities

• Assist patients and families to understand significant social and emotional factors related to health status.
• Contribute to an integrated Plan of Care for patients on a daily basis.
• Instruct care team members on community resources available to patients as needed.
• Serve as Bereavement Coordinator and develop a bereavement plan of care when required.
