Qualifications

• Master's Degree from a CSWE-accredited School of Social Work.
• 1+ years of social work experience in a health care setting.
• Current Driver's License, vehicle insurance, and access to dependable transportation.

Responsibilities

• Assist patient and family to understand social factors related to health status and coping.
• Contribute to a comprehensive Plan of Care with Interdisciplinary Team.
• Instruct team on community resources and bereavement services if needed.
