Job summary

Four Seasons Compassion for Life seeks a skilled Social Worker to provide psychosocial services to hospice patients and families. The role covers assessments, care planning, and coordination with community resources, ensuring compassionate support across care settings.

Responsibilities include discharge planning, family counseling, and collaboration with the IDG team. A BS(W/MSW) and 1–3 years in healthcare are required, with a valid driver’s license and flexible availability.

Qualifications

• Bachelor’s in Social Work (BSW) or Master’s in Social Work (MSW) required
• 1–3 years of healthcare experience preferred
• Valid driver’s license required
• Ability to perform psychosocial assessments and care planning

Responsibilities

• Perform initial and ongoing psychosocial assessments of patients and families
• Develop and evaluate plans of care for assigned patients
• Coordinate referrals with community health and social agencies
• Provide counseling and education to patients and families
• Assist with discharge planning and end-of-life care discussions
• Participate in IDG team meetings to review care plans
• Identify financial assistance needs and assist in obtaining resources
• Maintain accurate documentation in EMR and comply with regulations
• Support 24-hour on-call psychosocial services as needed
• Ensure safety and infection control practices
