Job summary

St. Croix Hospice in Minnesota is seeking a compassionate Social Worker to join the patient care team. You will meet with patients and families, coordinate with the clinical staff, and help access resources and care plans.

The role requires a CSWE-accredited BSW, current state licensure (LSW/LGSW/LISW/LICSW), reliable transportation, and ability to pass background checks. You’ll support end-of-life needs with counseling and bereavement services.

Qualifications

• Bachelor’s degree in social work from an institution accredited by the CSWE.
• Current Social Work license in practicing state(s) (LSW/LGSW/LISW/LICSW).
• Ability to pass background, consumer report, and drug test.
• Reliable transportation and valid driver’s license.

Responsibilities

• Conduct psychosocial assessments, identify safety or support needs, and provide resources, referrals, and care planning support for patients and families.
• Provide counseling, crisis intervention, and bereavement support, including follow up for at risk individuals and coordination of bereavement activities.
• Collaborate with the interdisciplinary team, maintain accurate documentation, support cultural and end of life needs, liaise with community agencies, and ensure HIPAA compliance.
