Job summary

Otterbein SeniorLife seeks a dedicated Hospice Social Worker to deliver comprehensive psychosocial services to terminally ill patients and families. PRN shifts available across Otterbein Hospice services, emphasizing compassionate, person-centered care and teamwork with our interdisciplinary staff.

The role requires a Master’s in social work, an active Ohio license, and at least 1 year in health care; hospice experience is preferred.

Qualifications

• Master’s degree in social work required.
• Active Ohio Social Work license required.
• Minimum 1 year experience in healthcare; hospice preferred.
• Strong communication and counseling skills required.

Responsibilities

• Conduct psychosocial assessments within five calendar days after patient election of Hospice care.
• Provide psychosocial counseling to patients, families and caregivers.
• Assist with placement planning for facilities for basic care or respite when appropriate.
• Develop and update social histories and plans of care; participate in IDT meetings.
• Coordinate referrals to community resources and financial assistance; ensure timely follow-up.
