Job summary

Otterbein SeniorLife seeks a compassionate Hospice Social Worker to provide psychosocial support to terminally ill patients and their families. The role emphasizes counseling, resource referral, and collaboration with the care team to ensure dignity and comfort at every stage of care.

Qualifications

• Master's degree in social work from a CSWE-accredited program.
• Active Ohio Social Work license.
• Valid driver's license and reliable transportation.

Responsibilities

• Conduct initial psychosocial and bereavement risk assessment within five days after Hospice election.
• Refer patients and families to community resources and financial assistance as needed.
• Assist family/caregiver in placement of patient in facility for basic care, respite or inpatient care as appropriate.
• Provide psychosocial and anticipatory grief counseling to patients, families, caregivers and significant others.
• Develop and update the plan of care; participate in IDT meetings to coordinate plans.
