Qualifications

• Bachelor's degree in nursing preferred.
• Graduate of approved nursing program.
• Valid Ohio RN license.
• 2-4 years direct patient care experience in hospice.
• Experience with end-of-life diagnosis preferred.

Responsibilities

• Primary caseload management for Hospice patients.
• Provide skilled professional nursing care to patients, families and caregivers as prescribed by the physician and in compliance with Hospice Medicare Conditions of Participation, Ohio licensure laws, and agency policies.
