Job summary

HealthPartners’ Olivia Hospital & Clinic is seeking a Hospice Social Worker to deliver psychosocial assessments, counseling, bereavement services, and community education as part of the hospice interdisciplinary team. The role is approximately 24 hours per week and requires Minnesota licensure (LGSW/LISW/LICSW) and a MSW.

Responsibilities include coordinating grief support, assisting with admissions, documenting in medical records, and providing staff education.

Qualifications

• Master's degree in social work is required.
• Current LGSW, LISW, or LICSW in Minnesota is required.
• At least 1 year of experience in social work or hospice settings is preferred.
• Excellent organizational and problem-solving skills are essential.
• Ability to read and interpret documentation and medical records.
• Basic computer skills with Excel, Outlook and Word required.
• Ability to manage stress and emotional challenges professionally.
• Ability to work independently per hospital values.
• Strong critical thinking and effective communication skills.

Responsibilities

• Performs psychosocial assessments to identify needs and determine service delivery.
• Contributes psychosocial input to the patient plan of care.
• Provides bereavement and grief support to families.
• Educates staff and volunteers on social work principles.
• Completes documentation for assessments and observations.
• Assists with admissions, crisis intervention, and discharge planning.
• Acts as liaison to community resources and supports referrals.
• Other duties as assigned by the hospice team.
