Job summary

Alpha Home Health and Hospice is seeking a part-time Hospice Social Worker (MSW/LICSW) to support patients in the greater Snohomish County area. The role focuses on psychosocial assessments, resource coordination, and plan-of-care development in patients' homes, with involvement in bereavement coordination and quality improvement initiatives.

Requirements include an accredited social work degree (MSW/LICSW preferred), at least one year in health care, and a valid driver's license with insured

Qualifications

• Graduate of an accredited social work program (MSW/LICSW preferred).
• Minimum of one year in healthcare; home health/hospice preferred.
• Licensed driver with insured vehicle; DMV in good standing.

Responsibilities

• Assess psychosocial status of patients and illness progression.
• Develop and coordinate community resources for home-based care.
• Assist in developing individualized plan of care with the team.
• Liaise with community agencies and coordinate bereavement support.
