Job summary

Vivie is seeking a Social Worker to provide psychosocial support to patients and families during end-of-life care, conducting assessments and coordinating support services. The role includes bereavement care, advocacy for patients and families, and ensuring compliance with hospice policies.

The position requires a Bachelor’s degree in Social Work and a Minnesota license is preferred; hospice or geriatric care experience is desirable.

Qualifications

• Bachelor's degree in Social Work required.
• Minnesota Social Work License preferred; if licensed, must maintain CE requirements.
• Experience in hospice, healthcare, or geriatric care preferred.

Responsibilities

• Provide psychosocial support to patients and families, including home visits and assessments.
• Act as an IDG team member; participate in care conferences and document updates.
• Provide bereavement and emotional support; coordinate with Bereavement Team.
