Hospice of the Midwest in Forest Lake, MN, seeks a dedicated Social Worker to provide emotional support, counseling, and resource coordination to patients facing terminal illness. The role emphasizes holistic care and dignity at the end of life.

Qualifications

• MSW or BS in Social Work per CSWE accreditation.
• Minimum 1 year healthcare/hospice experience preferred.
• Hospice philosophy understanding and issues of death/dying.
• Current licensure or good standing per state guidelines.
• Licensed driver with insured vehicle.

Responsibilities

• Assess psychosocial needs of patients and families.
• Evaluate home safety and assist with care plans.
• Provide counseling, crisis intervention & emotional support.
• Maintain accurate and timely clinical documentation.
• Educate and assist with advance directives.
• Coordinate referrals with community resources.
