Qualifications

Responsibilities

• Assess psychosocial status of patients and families and inform the care team.
• Conduct social evaluations including family dynamics and caregiver abilities.
• Educate on advance directives and connect with community resources.
• Maintain clinical records and progress notes.
• Oversee bereavement and volunteer programs.
