Job summary

The ideal candidate holds an LCSW or LSW licensure, a Master’s in Social Work, and relevant hospice experience, with a valid driver’s license and proof of auto insurance.

Qualifications

• Licensed Clinical Social Worker (LCSW) license required.
• Driver's license and auto insurance required.
• Licensed Social Worker (LSW) license preferred/required per jurisdiction.
• Master's Degree in Social Work required; hospice experience preferred.

Responsibilities

• Conduct psychosocial assessments of patients and families to identify emotional and social needs.
• Develop bereavement plans and support for family members/caregivers.
• Assist with transportation barriers and referrals for community resources.
• Document assessments and interventions in the patient’s EMR and communicate with the IDT.
• Support patient decisions and respect cultural and spiritual beliefs in care planning.
