Job summary

Compassus is seeking a Hospice Social Worker to model our values and provide psychosocial care to patients and families, coordinating with the IDT to plan, implement, and adjust care plans. The role requires an MSW from a CSWE-accredited program with healthcare experience and appropriate state licensure.

Qualifications

• MSW from CSWE-accredited program with at least 1 year of healthcare experience.
• Bachelor's degree in social work with 1 year of healthcare experience may be considered for locations with MSW supervision.
• Must hold state licensure as required for the state where you practice.

Responsibilities

• Explain hospice services and Medicare benefits to patients and families; obtain informed consent and benefit election documents.
• Submit documentation to facilities per policy after patient visits.
• Participate in the Interdisciplinary Team (IDT) and care plan development and implementation.
• Provide psycho-social support and counseling to patients and families; coordinate external counseling as needed.
• Identify need for intervention by other IDT members and communicate needs clearly.
• Coordinate psycho-social care with IDT to ensure continuity and quality of care.
• Maintain updated care plans reflecting problems, goals, interventions.
