Qualifications

• Master’s Degree from an accredited school of Social Work with licensure if required.
• Medicare/Medicaid/Insurance knowledge preferred.
• 1+ year post-graduate social work in healthcare; 2+ years preferred.

Responsibilities

• Assist core hospice team in understanding social, spiritual, and emotional factors to establish a plan of care.
• Participate as a member of the interdisciplinary team and review the plan of care.
• Assess social, spiritual, and emotional needs to estimate capacity to cope with illness and daily living.
• Prepare the patient to cope with changes and educate family on advance directives.
• Utilize resources to help patient and family live better within illness limits.
• Support the bereavement program.
• Provide discharge planning related to level-of-care changes.
