Job summary

Fairview Range in Hibbing, MN is seeking a Social Worker (MSW) to join our Home Care and Hospice Team. The role involves direct service to Hospice and Home Care clients and collaboration with the interdisciplinary team to support patient and family needs.

A Master of Social Work and MN licensure are required; a valid MN driver's license is necessary. The position may involve bereavement support and coordination with RNs and community resources.

Qualifications

• Master of Social Work degree required.
• Current MN Social Worker licensure required.
• Valid MN driver's license and auto insurance required.

Responsibilities

• Assess and plan interventions for patient and family needs.
• Explain hospice criteria to patient/family and refer sources as needed.
• Advocate for patient and family throughout care.
• Explain hospice coverage under Medicare, Medicaid and Private Insurances; obtain prior authorization as needed.
• Coordinate with RN case manager on all aspects of hospice care.
