Job summary

Carle Health seeks a Home Services Social Worker to assess psychosocial needs, coordinate resources, and provide short-term counseling within an interdisciplinary team. Hospice experience of 1+ years is required, and a sign-on bonus is available.

Responsibilities include conducting psychosocial assessments, developing bereavement plans, and ensuring documentation in the EMR. Travel to patient homes may be required based on census and program needs.

Qualifications

• Hospice experience of 1+ years required.
• Ability to work as part of an interdisciplinary team.
• MSW degree with relevant licensure desired.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develops a bereavement plan of care to address family needs.
• Interprets and communicates pt/family faith and culture traditions.
• Educates patient and family members to overcome barriers and meet basic needs.
• Documents psychosocial assessments and MSW interventions in the EMR.
