Job summary

Affinity Health Management is seeking a licensed Master’s Social Worker (MSW) to implement standards of care for hospice social work services provided to patients and families. The MSW is a core member of the Interdisciplinary Group and delivers psychosocial support based on assessments and individualized plan-of-care goals.

Qualifications

• Master’s degree in Social Work (MSW) required.
• Minimum 1 year of health care experience; hospice experience preferred.
• Strong verbal and written communication skills.
• Ability to assess psychosocial needs and develop interventions.
• Knowledge of culture-sensitive care and advanced directives.
• Experience in crisis intervention and patient/family counseling.
• Must be able to drive and have a valid driver’s license.
• CPR certification required (hands-on validation preferred).

Responsibilities

• Assess psychosocial status of patients and families and communicate findings to the care team.
• Conduct social evaluations including family dynamics and caregiver abilities.
• Educate interdisciplinary group on cultural needs and advanced directives.
• Assist in development and monitoring of the care plan.
• Support funeral planning, financial, legal, and health care decision responsibilities.
• Provide crisis intervention and continuity of care in collaboration with hospice team.
• Maintain accurate clinical notes and coordinate with physicians and community agencies.
