Qualifications

• Master’s Degree from a school of Social Work accredited by the Council on Social Work Education.
• Current Driver’s License with vehicle access.
• CPR certification or ability to obtain within 90 days of hire.
• Licensed Social Worker in the state of residence.

Responsibilities

• Assist the patient and family to understand significant personal, emotional, environmental and social factors related to health.
• Contribute to development of a comprehensive Plan of Care with the interdisciplinary team.
• Instruct staff on community resources available to support patients.

Education

Master’s Degree in Social Work (CSWE accredited)

Licensed Social Worker in the state of residence
