National Church Residences in southern Ohio is seeking a Medical Social Worker to join our Home Health division. This full-time role includes a $5,000 sign-on bonus and an on-call rotation.

Qualified candidates hold a CSWE-accredited BSW/MSW with an LSW in good standing and experience in healthcare settings. You will collaborate with a multidisciplinary team to support seniors and families in our community.

Qualifications

• Bachelor’s or MSW in CSWE-accredited program.
• LSW in good standing preferred.
• Experience in healthcare settings preferred.

Responsabilités

• Provide social work services to seniors and families.
• Collaborate with health care teammates for care planning.
• Assist with patient advocacy and discharge planning.
