Job summary

Buckeye Hospice and Palliative Care Services is seeking a Medical Social Worker to provide psychosocial care to patients and families as part of the plan of care. This role requires completing a psychosocial assessment within 5 days of admission, participating in IDG meetings, and maintaining thorough clinical records.

Qualifications

• Must have current social work licensure in Ohio
• MSW preferred; BSW with supervision acceptable
• Experience in healthcare or hospice settings is preferred

Responsibilities

• Provide psychosocial support to patients and families as part of the plan of care
• Conduct psychosocial assessments within 5 days of admission
• Attend IDG meetings and contribute to care planning
• Document initial evaluation, care plans and daily notes
• Assist with discharge planning and coordination with Case Managers
• Support patients and families in coping with illness and dying
• Maintain professional records and ensure timely reporting
