Job summary

Buckeye Hospice and Palliative Care Services in Ohio is seeking a dedicated Medical Social Worker to provide psychosocial support to patients and families under the plan of care. You will conduct timely assessments, participate in IDG meetings, and help coordinate care with the broader health team.

The role requires OH licensure, MSW or BSWS with supervision, and prior hospice experience is preferred. A valid driver’s license and strong written/verbal communication are essential.

Qualifications

• Licensed social worker with OH licensure.
• MSW degree from CSWE-accredited program; or BSW with MSW supervision.
• Hospice experience preferred.
• Valid driver's license with insured vehicle.
• Excellent verbal and written communication skills.

Responsibilities

• Provide Medical Social Services to patients and families per Plan of Care.
• Complete psychosocial assessment within 5 days of admission.
• Participate in IDG meetings and develop patient care plans.
• Record and report changes in emotional/social status.
• Maintain written clinical records: evaluation, care plan, daily notes.
• Evaluate patient/family response to social work interventions.
• Coordinate visits with Case Managers for scheduling.
• Participate in IDG and QAPI meetings.
• Assist in discharge planning and patient/family coping with dying.
