Buckeye Hospice and Palliative Care Services is seeking a Medical Social Worker to provide psychosocial assessment and ongoing support for patients and families under the hospice program. The role involves collaboration with the interdisciplinary group, care planning, and caregiver support in home settings across West Union, OH.

The position requires an Ohio social work license (MSW or BSW with experience) and a valid driver’s license.

Qualifications

• Must be licensed to practice social work in Ohio.
• MSW degree required; CSWE-accredited; BSW with 1 year healthcare social work experience acceptable.
• Hospice experience preferred; valid driver's license required.

Responsabilités

• Conduct psychosocial assessment within 5 days of admission to inform the patient’s plan of care.
• Participate in IDG meetings and develop the patient’s plan of care.
• Maintain and submit written clinical records including evaluation, care plan and daily notes.
• Coordinate visits with Case Managers and other staff; participate in discharge planning as needed.
• Assist patients and families with social, emotional and environmental needs related to illness and end-of-life care.
