Job summary

Buckeye Hospice and Palliative Care Services in Greenfield, Ohio, seeks a compassionate Social Worker to deliver emotional, social, and financial support to patients and families at a difficult time. You will collaborate with an interdisciplinary team to develop care plans and advocate for patients’ needs.

The role requires MSW (CSWE) or a BSW with healthcare experience, Ohio licensure, and strong communication and teamwork skills.

Qualifications

• MSW from CSWE-accredited program or BSW with 1 year healthcare social work experience.
• Licensed to practice social work in Ohio (LSW/LCSW/LISW).
• Hospice experience preferred.
• Ability to communicate effectively with patients and families.
• Ability to work independently and as part of a team.

Responsibilities

• Provide Medical Social Services per Plan of Care.
• Conduct psychosocial assessments and develop interventions.
• Participate in IDG meetings and care planning.
• Provide emotional support and counseling to patients and families.
• Identify gaps in care and refer to community resources.
• Maintain accurate patient records and collaborate with clinicians.
