Job summary

Silverado Hospice Los Angeles is seeking a compassionate MSW Social Worker to provide emotional support, counseling, and resource coordination for patients and families on the hospice journey. The position is PRN with a Monday–Friday, fully variable schedule in the Los Angeles area.

Anticipated pay range is $39–$41 USD, with benefits including 401(k), health/dental/vision, FSA, PTO, tuition reimbursement, referral programs, and more.

Qualifications

  • Master’s degree in Social Work required
  • Documented evidence of training in grief and loss counseling
  • Valid driver’s license, good driving record, and reliable transportation

Responsibilities

  • Conduct psychosocial assessments, develop care plans, and document bereavement activities
  • Design and distribute educational materials for families receiving bereavement services
  • Develop programs on grief and loss for families and staff
  • Evaluate the bereavement program quarterly and annually
  • Provide community consultation and education on hospice services
  • Maintain ongoing education through academic studies and workshops
  • Attend in-services, staff meetings, and case conferences
  • Report patient condition changes to the RN Case Manager
  • Observe and address safety hazards in patient and organizational environments
  • Assist the health team in understanding emotional factors related to terminal illness
  • Assess suicide risk, mental capacity, and elder/dependent adult abuse
  • Aid in advance directives, mortuary arrangements, and financial planning
  • Provide information and referrals to community resources and legal entities
  • Advise and counsel staff, patients, families, or agencies in emergency situations

Shift Schedule: PRN (Per Diem), Monday - Friday, fully variable

Anticipated pay range

$39 - $41 USD
