Job summary

Silverado Hospice in San Diego is seeking a Social Worker (MSW) to provide emotional support, counseling, and resource coordination for patients and families during the hospice journey. The role emphasizes grief counseling, education for families, and collaboration with the health team in a compassionate, mission-driven environment.

• Documented evidence of training in grief and loss counseling
• Valid driver’s license, good driving record, and reliable transportation

Responsibilities

• Conduct psychosocial assessments, develop care plans, and document bereavement activities
• Design and distribute educational materials for families receiving bereavement services
• Develop programs on grief and loss for families and staff
• Evaluate the bereavement program quarterly and annually
• Provide community consultation and education on hospice services
• Maintain ongoing education through academic studies and workshops
• Attend in-services, staff meetings, and case conferences
• Report patient condition changes to the RN Case Manager
• Observe and address safety hazards in patient and organizational environments
• Assist the health team in understanding emotional factors related to terminal illness
• Assess suicide risk, mental capacity, and elder/dependent adult abuse
• Aid in advance directives, mortuary arrangements, and financial planning
• Provide information and referrals to community resources and legal entities
• Advise and counsel staff, patients, families, or agencies in emergency situations

Shift Schedule: Part Time, Monday - Thursday, 8:30 AM - 5:00 PM, must have an open availability for weekend on-call rotation

Anticipated pay range: $35 USD - $39 USD
