Job summary

Silverado is hiring a Social Worker (MSW) in the Temecula area to provide emotional support, counseling, and resource coordination for patients and families on the palliative journey. You will conduct psychosocial assessments, develop care plans, and deliver bereavement education while collaborating with a multidisciplinary team in a nationally recognized workplace.

Shift is Monday–Friday, 8:00am–5:00pm with competitive pay and benefits including retirement, health, and professional development

Qualifications

  • Master’s degree in Social Work required.
  • Evidence of training in grief and loss counseling.
  • Valid driver’s license with reliable transportation.
  • Comfortable working with pets.

Responsibilities

  • Provide emotional support, counseling, and resource coordination for patients and families.
  • Conduct psychosocial assessments and develop care plans.
  • Design and distribute bereavement materials for families.
  • Develop grief and loss programs for families and staff.
  • Coordinate bereavement program evaluation and reporting.
  • Offer community education on hospice services.
  • Attend in-services, staff meetings, and case conferences.
  • Report changes to the RN Case Manager and address safety hazards.
  • Assist with advance directives, mortuary arrangements, and financial planning.
  • Provide information and referrals to community resources and agencies.

We’re hiring a Social Worker (MSW) – a compassionate and skilled professional who will provide emotional support, counseling, and resource coordination for patients and families during their palliative journey.

What You’ll Be Doing
  • Conduct psychosocial assessments, develop care plans, and document bereavement activities
  • Design and distribute educational materials for families receiving bereavement services
  • Develop programs on grief and loss for families and staff
  • Evaluate the bereavement program quarterly and annually
  • Provide community consultation and education on hospice services
  • Maintain ongoing education through academic studies and workshops
  • Attend in-services, staff meetings, and case conferences
  • Report patient condition changes to the RN Case Manager
  • Observe and address safety hazards in patient and organizational environments
  • Assist the health team in understanding emotional factors related to terminal illness
  • Assess suicide risk, mental capacity, and elder/dependent adult abuse
  • Aid in advance directives, mortuary arrangements, and financial planning
  • Provide information and referrals to community resources and legal entities
  • Advise and counsel staff, patients, families, or agencies in emergency situations
Qualifications
  • Master’s degree in Social Work required
  • Documented evidence of training in grief and loss counseling
  • Valid driver’s license, good driving record, and reliable transportation
  • Comfortable working in environments with pets

Shift Schedule: Monday - Friday, 8:00am - 5:00pm

Anticipated pay range

$35 - $40 USD
