Position Overview

The ConcertoCare Social Worker is a core member of the interdisciplinary team responsible for providing integrated medical, behavioral, and social care to patients with complex health care needs. They provide clinical care to medically and socially complex patients in community settings through in-person and telephonic visits, ensuring biopsychosocial needs are met, conducting behavioral health assessments, and engaging patients, caregivers, and the care team in treatment planning and coordination. They also provide brief evidence-based behavioral health therapies and crisis intervention, collaborating with other health professionals to ensure equitable behavioral health care.

Essential Duties and Responsibilities

• Support behavioral health needs of patients on caseload using evidence-based models
• Provide consultation to group residences to recognize behavioral health changes and support interventions
• Support psychotropic medication management in collaboration with prescribing providers
• Participate in caseload reviews and team meetings, communicate effectively about patient needs
• Complete timely assessments and document encounters in electronic health records
• Support treatment plan adjustments based on patient progress
• Serve as a subject matter expert in behavioral health care
• Ensure care respects patient diversity and individual needs
• Other duties as assigned

Qualifications

• Master’s degree in Social Work
• Current New York LCSW required
• 2+ years healthcare/behavioral health experience
• Geriatric care experience highly desired
• Strong communication, organizational, and multitasking skills
• Proficient in Microsoft Office and EMR systems
• Reliable internet connection
• High professionalism

Compensation

Base Salary/Wage Range: $60,000 to $80,000 plus annual bonus. Compensation is commensurate with experience and qualifications. The role offers a comprehensive benefits package including healthcare, 401K with match, and other wellness and financial benefits.
