Job description

Hospice Social Worker $53,000-$57,000 annually A Great Opportunity / Full-time At ViaQuest Hospice we take a holistic approach, offering services to treat the physical, emotional and mental needs of our patients and their loved ones. Responsibilities may include:

• Performing comprehensive patient assessments.
• Providing patient visits as scheduled.
• Providing short term crisis intervention and individual or family counseling as needed.
• Maintain accurate patient records and progress notes using our new nVoq system, which allows you to dictate notes effortlessly—spending more time with patients, not paperwork.
• Serving as a patient and family unit advocate.

Requirements for this position include:

• Masters degree in Social Work preferred, Bachelors in Social Work accepted.
• License in Social Work from State of Pennsylvania preferred.
• Experience in Medical Social Work is preferred.
• Experience within the Hospice field is preferred.
• Valid driver's license, car insurance and reliable transportation.
• Willingness to travel throughout assigned service area.
