Job summary

VitalCaring Group is seeking a Hospice Social Worker to support patients and families facing terminal illness with emotional support, resource coordination, crisis intervention, and compassionate guidance.

The role requires a Master’s degree in Social Work, preferably with hospice experience. You will work with the interdisciplinary team to ensure dignity, comfort, and holistic care, including possible home visits by a licensed driver with insurance.

Qualifications

• Master’s degree in Social Work (MSW).
• One year of social work experience in a hospice setting preferred; strong understanding of hospice philosophy and end-of-life issues.
• Ability to work tactfully with patients, families, and the community.
• If performing home visits: must be a licensed driver with an insured, reliable automobile.

Responsibilities

• Psychosocial Assessment & Support: assess psychosocial status, communicate findings to RN and the team; provide counseling and crisis intervention.
• Resource Coordination: provide information, referrals, and assist with funeral planning, financial and legal matters.
• Interdisciplinary Collaboration: participate in care planning and IDG meetings, support team understanding of social factors.
• Communication & Documentation: maintain accurate notes and liaison with community agencies to ensure seamless care.
• Additional Responsibilities: participate in quality improvement activities; perform duties as directed by Branch Director.
