Job summary

Moments Hospice seeks a compassionate Licensed Social Worker to deliver psychosocial services to terminally ill patients and families. You will collaborate with the care team to ensure timely assessments and support, including bereavement planning where appropriate.

This role emphasizes autonomy, on-call support, and direct leadership collaboration. A master’s or social work degree and active state license are required, with hospice experience a plus.

Qualifications

• Master’s degree in Social Work preferred.
• Bachelor's degree in Social Work with a minimum of 1 year of healthcare experience.
• Two years social work experience in a healthcare setting or social services agency preferred.
• Current Social Worker license is required.
• Hospice experience is a plus.
• Knowledge of community resources and eligibility requirements.

Responsibilities

• Provide psychosocial services to terminally ill patients and their families.
• Make contact with patient, family, and caregiver within 48 hours of admission.
• Perform Case Manager duties and psychosocial assessments within 24 hours of completion.
• Regularly assess patients and family psychosocial needs and pre-bereavement/bereavement care.
• Participate in weekly interdisciplinary team meetings and assist with funeral arrangements if requested.
