The role emphasizes psychosocial assessment, care coordination, and collaboration with the interdisciplinary team to ensure seamless support across the care continuum.

Qualifications

• MSW required; hospice knowledge preferred.
• One year of hospice social work experience preferred.
• Ability to work tactfully with patients, families, and community.
• If home visits: must have a valid driver's license and insured vehicle.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Coordinate resources, referrals, and services to support care planning.
• Participate in interdisciplinary team meetings to align psychosocial care.
• Maintain thorough documentation of progress notes and care planning.
• Support quality assessment and performance improvement activities.
