Qualifications

• MSW required or equivalent.
• One year hospice social work experience preferred.
• Licensed driver with insured vehicle for home visits.

Responsibilities

• Assess the psychosocial status of patients and families; communicate findings to the RN and IDG.
• Provide counseling to patients and families experiencing stress, grief, and coping challenges.
• Coordinate information, referrals, and resources for practical and environmental needs.
