Job summary

Altus Hospice is seeking a dedicated Licensed Master Social Worker (LMSW) to provide compassionate social work services to hospice patients and families in Victoria, TX. The per diem position emphasizes counseling, psychosocial assessments, and advocacy within an interdisciplinary team.

Responsibilities include coordinating care plans, connecting families with resources, and supporting bereavement services, with a focus on dignity, peace, and human connection during end-of-life care.

Qualifications

• Master of Social Work from an accredited school
• Licensed Social Worker by the State Board of Social Work
• Minimum 1 year hospice social work experience; healthcare setting 1 year preferred

Responsibilities

• Assess psychosocial status and communicate findings to the RN Case Manager and interdisciplinary team
• Evaluate home situations, financial resources, and community support for patients
• Develop and implement individualized social work plans in collaboration with Bereavement Coordinator
• Contribute to care planning by attending interdisciplinary meetings and addressing psychosocial stresses
• Provide empathetic counseling to patients and families during end-of-life care
