Crown Hospice in Victoria, TX is hiring a Licensed Hospice Social Worker (LBSW, LMSW, or LCSW) to join our interdisciplinary team. The role focuses on compassionate counseling, psychosocial assessments, and resource coordination for patients and families facing end-of-life care.

Qualifications

• MSW from an accredited program
• Current Texas MSW license
• Minimum 1 year healthcare experience (hospice/home health preferred)
• Strong communication, organization, and relationship-building skills
• Current CPR certification or willingness to obtain
• Valid driver’s license, reliable transportation, and automobile insurance

Responsibilities

• Provide emotional support counseling to patients and families navigating end-of-life care.
• Complete psychosocial assessments and develop individualized plans.
• Connect patients and families with community resources, advance care planning, and financial resources.
• Advocate, educate, and guide families throughout the hospice journey.
• Collaborate with nurses, physicians, chaplains, aides, and other team members to provide holistic care.
