Job summary

Altus Hospice is seeking a dedicated Licensed Master Social Worker (LMSW) to provide compassionate social work services to hospice patients and families in Victoria. You will assess psychosocial needs, offer counseling, and collaborate with the care team to support emotional well-being.

Requirements include a Master of Social Work, state licensure, a valid driving license, and healthcare experience; hospice experience preferred.

Qualifications

• Master of Social Work from an accredited school of social work.
• Licensed as a Social Worker by the State Board of Social Work.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle.
• Minimum one year of healthcare social work experience; hospice or long-term care preferred.

Responsibilities

• Be a supportive guide: assess and communicate psychosocial status to the RN Case Manager and interdisciplinary team.
• Connect Hearts and Homes: evaluate medical needs, home situations, financial resources and community support.
• Provide individualized care advocacy: implement social work plans and coordinate with Bereavement Coordinator.
• Be part of a united team: participate in care plan development and interdisciplinary meetings addressing psychosocial stresses.
• Provide counseling with compassion to patients and families.
