Qualifications

Responsabilités

• Complete and document comprehensive psychosocial assessments
• Identify care planning goals and interventions to improve outcomes
• Participate in Interdisciplinary Team meetings and provide psychosocial insight
• Provide education on funeral planning and advanced directives
• Visit patients and caregivers in their homes or resident settings
• Offer bereavement support to caregivers after a patient’s death
• Participate in on-call rotation for after-hours visits during death or funeral events
