Job summary

Caris Healthcare is seeking a Social Worker (MSW) to support the hospice interdisciplinary team by performing comprehensive psychosocial assessments, guiding care planning, and providing emotional support to patients and caregivers at end of life.

The role involves visiting patients in their residences, offering bereavement support, and collaborating in an on-call rotation. Relevant licensure and experience are required or preferred per state guidelines.

Qualifications

• MSW preferred; BSW required.
• SC licensure required; LMSW preferred.
• One year of social work experience in a health care setting.
• Excellent communication and interpersonal skills.

Responsibilities

• Completes and documents comprehensive psychosocial assessments with patients in the EMR.
• Identifies care planning goals and develops interventions to promote best outcomes.
• Participates in Interdisciplinary Team meetings; provides psychosocial and emotional insights.
• Provides education related to funeral planning and advanced directives as needed.
• Conducts visits with the patient and caregiver in their home setting or residence.
• Assists with bereavement support and interventions for caregivers after a patient’s death.
• Participates in an on-call rotation for after-hours visits as needed.
