Amedisys is seeking a Social Worker (MSW/LSW) for hospice care in a full-time, first-shift role. You will support patients and families with psychosocial needs, coordinate with the care team, and help navigate financial and practical barriers to care at home.

Qualificações

• Holds a masters or doctorate degree in social work from a school accredited by the Council on Social Work Education.
• Meets state and federal requirements for this position.

Responsabilidades

• Assists the patient and family with health related financial, social and emotional concerns and assists with effective coping skills.
• Provides social work services in accordance with the plan of care.
• Ensures changes in the patient's clinical status are communicated to other health care team members and care center staff in a timely manner.
• Educates patient and family and other team members on social work interventions and treatment plan.
• Performs care planning, transfer and discharge functions.
• Identifies risks in the patient’s environment and implements interventions to reduce risks, communicating with physician and team.
