Qualifications

• MSW degree from CSWE-accredited program and LMSW license required.
• Hospice or palliative care experience preferred.
• Two years of verifiable experience as a Social Worker preferred.

Responsibilities

• Assess patients' psychosocial needs and complete social histories.
• Document care plans, progress, and follow-up per regulations.
• Provide counseling and emotional support to residents and families.
• Coordinate referrals and act as liaison with community resources.
• Partner with the care team to enhance quality of life and outcomes.
