Job summary

Care Initiatives Hospice in Iowa is seeking a compassionate Social Worker to provide holistic, patient- and family-centered care during life-limiting illnesses. You will meet patients where they are—at home, in long-term care, or in hospital—delivering dignity and support.

As part of a dedicated team in Iowa Falls and surrounding communities, you will assess psychosocial needs, coordinate referrals, and collaborate with caregivers to enhance quality of life while honoring patients' wishes.

Qualifications

• Bachelor's Degree in Social Work, required.
• Hospice or palliative care experience, preferred.
• Two years of verifiable experience working as a Social Worker, preferred.
• Current driver's license, automobile liability insurance, and ability to travel between locations within your region.

Responsibilities

• Assess patients’ psychosocial needs and complete social histories
• Document care plans, progress, and follow-up per regulations
• Provide counseling and emotional support to residents and families
• Support patients and families with adjustment, rights, and orientation
• Coordinate referrals and act as a liaison with community resources
• Partner with the care team to enhance quality of life and outcomes
• Plan and support patient, family, and community engagement activities
• Promote facility programs and services to referral partners
