Care Initiatives Hospice is seeking a Social Worker to deliver comprehensive psychosocial care to patients and families in Iowa. The role supports residents across home, long-term care, assisted living, or hospital settings with a compassionate, holistic approach.

You will assess psychosocial needs, document care plans, provide counseling, and collaborate with the care team to improve quality of life. A BS in Social Work and driving ability are required, with hospice experience preferred.

Qualifications

• Bachelor's degree in Social Work is required.
• Hospice or palliative care experience preferred.
• Two years of verifiable experience as a Social Worker preferred.
• Current driver's license and ability to travel between multiple locations within region.

• Document care plans, progress, and follow-up per regulations.
• Provide counseling and emotional support to residents and families.
• Support patients and families with adjustment, rights, and orientation.

Care Initiatives Hospice is looking for a Social Worker who is located in Iowa Falls, Hampton, Eldora and surrounding areas.

What it takes to be a Social Worker at Care Initiatives Hospice

• Bachelors Degree in Social Work, required.
• Hospice or palliative care experience, preferred.
• Two (2) years of verifiable experience working as a Social Worker, preferred.
• Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region.
