Job summary

Baptist Memorial Hospice in Tennessee, part of the Optum family, is seeking a Social Worker to provide medical social services under a physician and interdisciplinary group. You will assess social and emotional factors affecting health, help develop an integrated Plan of Care, and guide the care team in using community resources.

Requirements include an MSW from a CSWE-accredited program and a valid driver's license; CPR and state licensure are preferred; 1+ year healthcare social work

Qualifications

• Master's Degree from CSWE-accredited school of Social Work.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.
• Licensed Social Worker in the state of residence.
• 1+ years of social work experience in a health care setting.

Responsibilities

• Assists the patient, significant others, physician and health care team staff to understand significant personal, emotional, environmental and social factors related to the patient's health status on an as needed basis.
• Contributes as a health care team member to the development of a comprehensive, integrated Plan of Care for patients on a daily basis.
• Instructs health care team members on community resources available to assist patients on a as needed basis.
