Community Hospice, part of LHC Group and the Optum family, is seeking a Social Worker in Tupelo, Mississippi. You will provide medical social services under physician direction to help patients cope with health-related challenges and coordinate with the Interdisciplinary Group.

Responsibilities include supporting patients and families, developing an integrated Plan of Care, and guiding the team to access community resources. A valid MSW degree and reliable transportation are required.

Formación

• Master's Degree in Social Work from an accredited school (CSWE) is required.
• Current driver's license, vehicle insurance, and access to reliable transportation.

Responsabilidades

• Assist patient and family to understand social and emotional factors related to health status.
• Contribute to a comprehensive Plan of Care in collaboration with the health care team.
• Educate team members on community resources available to support patients.
