Job summary

Heart’n Soul Hospice in Memphis, TN seeks a Social Worker to provide psychosocial assessment, counseling, and support for patients and families facing life-limiting illness.

Requires LMSW with state licensure; LCSW preferred. Must have a master’s in social work and a valid driver's license. One year of clinical social work experience is preferred. Includes full-time benefits and on-call duties.

Qualifications

• Master’s degree in social work from a State accredited program.
• Current State license required in accordance with State regulations.
• Current Driver's License, reliable transportation, and ongoing auto liability insurance.
• One year of experience as a Social Worker in a clinical care setting preferred.

Responsibilities

• Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs.
• Provide individual patient/family counseling, crisis intervention, and assistance with advance directives.
• Facilitate community resources based on patient/family needs.
• Accept clinical assignments consistent with education and competence.
• Perform on-call responsibilities and timely documentation, while ensuring compliance with laws and policies.
